The San Francisco Department of Public Health (DPH) is accepting applications for full-time temporary exempt positions in class 2586 Health Worker 2. The current positions are for the San Francisco Health Network Community Primary Care (SFHN CPC), Primary Care Central Team.
The applicant pool from this recruitment may be used fill future TEX positions at other locations within DPH.
The SFHN CPC consists of more than 10 community-based sites in San Francisco that provide high-quality, compassionate care across the lifespan and offer onsite dental, nutrition, pharmacy, and podiatry services, as well as integrative medicine services such as acupuncture.
Positions may be allocated to support on-site at any San Francisco Health Network primary care clinic including but not limited to the Castro-Mission Health Center, Children’s Health Center, Chinatown Public Health Center, Community Health Programs for Youth, Curry Senior Center, Family Health Center, Maxine Hall Health Center, Ocean Park Health Center, Positive Health Program, Potrero Hill Health Center, Richard Fine People’s Clinic, Southeast Health Center, Tom Waddell Urban Health Center, or Special Programs for Youth, as directed by staffing needs.
Shift: Monday - Friday, 8 AM - 5 PM; depending on the needs of the department evening and weekend shifts may be required.
Role description
Under the direction of the central team Health Program Coordinator, clinic site Medical Directors, and Practice Managers, the 2586 Health Worker 2 performs the following duties:
- Perform high volume of outreach and referral services to aid in chronic disease management and preventive screening;
- Manage electronic health registries for key populations (i.e. disease);
- Make appointments for patients and answer questions from clients;
- Maintain accurate logs and other documentation related to appointment processing;
- Perform incidental clerical functions such as keeping records, and arranging client appointments;
- Refer clients to appropriate Medical Home or clinical support as needed;
- Coordinate appointment making processes with the various Medical Homes;
- Provide information and resources to patients and others regarding health care services and other available facilities;
- Serve as a liaison between the professional staff and community;
- Assist in the preparation and dissemination of health services information;
- Participates in the development and coordination of programs, services, and studies;
- Prepares, and updates written documents;
- Simultaneously utilizes multiple computer systems and telephone systems to document in the electronic medical record;
- Adheres to established scripts and protocols to assist callers with various clinical and scheduling needs;
- Provides health coaching, per protocols to SF Health Network patients over the telephone;
- Uses electronic medical record to communicate to health care clinic staff and providers; and
- Proactively elevates problems and offers solutions.
The 2586 Health Worker 2 performs other related duties as assigned/required.
How to qualify
Experience: One (1) year of verifiable experience within the last five (5) years, performing a combination of at least two (2) of the following duties:
- Serving as a liaison between targeted communities and healthcare agencies;
- providing culturally appropriate health education/information and outreach to targeted populations;
- providing referral and follow up services or otherwise coordinating care;
- providing informal counseling, social support and advocacy to targeted populations;
- escorting and transporting clients;
- providing courier/dispatcher functions;
- performing pre-clinical examinations of vital statistics, such as measuring a patient’s weight, height, temperature and blood pressure.
Substitution: Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience.
One year of full-time employment is equivalent to 2000 hours. (2000 hours of qualifying work experience is based on a 40-hour work week.) Any overtime hours that you work above forty (40) hours per week are not included in the calculation to determine full-time employment.

- Note on Electronic Health Record (EHR): The Department of Public Health (DPH) is implementing a unified Electronic Health Record (EHR) system and DPH employees must demonstrate competency in the use of the system that is appropriate for their classification as a condition of employment.
- Note on Personal Protective Equipment (PPE): Some positions in the Department of Public Health will require the use of personal protective equipment (PPE), including but not limited to gloves, gowns, eye and face protection, and face-fitting respirators. The requirement for the use of PPE may come on short or no notice. Facial hair or any condition that interferes with a face-fitting respirator’s seal (i.e. comes between the sealing surfaces of the respirator and the wearer’s bare skin) is not permitted when face-fitting respirators are worn, including during initial or periodic respirator fit-testing.Employees who choose not to shave and do not have either American Disabilities Act (ADA) or Equal Employment Opportunity (EEO) Accommodations do not have the right to alternate work assignments or the option of using a loose-fitting Powered Air Purifying Respirator (PAPR) in place of a FaceFitting Respirator.
