This is a Permanent Exempt (PEX) recruitment conducted in accordance with City Charter Section 10.104 and is exempt from Civil Service.

The San Francisco Department of Public Health is accepting applications for a permanent exempt Medical Director (1165), Laguna Honda Hospital.

The Mission of the San Francisco Department of Public Health (SFDPH) is to protect and promote the health of all San Franciscans.

The San Francisco Health Network is the City’s only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers.

Role description

This position functions as a top executive for Laguna Honda Hospital and Rehabilitation Center (LHH) and is the top senior level executive responsible for medical care. This position is the accountable leader for medical care standards, coordination, surveillance and plans for improvement. This role will report to the Chief Executive Officer/Nursing Home Administrator (NHA).  This role will lead, guide, and direct the medical staff and operations of the healthcare facility in accordance with local, state, and federal regulations, standards and established facility policies and procedures to provide appropriate care and services to residents.  

They also provide leadership and direction in developing new clinical programs and establishing organizational policies, priorities, and objectives; directs and coordinates the implementation of program changes; serves as technical advisor; maintaining state and federal regulatory compliance; and performs related duties as required.  They make decisions which directly impact the entire facility. This position exercises supervision over LHH related functional areas.   

Specifically, under general direction from the NHA, the Medical Director will:

  • Know current standards of practice related to the care of long-term care residents and how to oversee other practitioners.  

  • Participate in administrative decisions related to resident care policies, including developing, recommending, and approving these policies.  

  • Participate on the Quality Assurance and Performance Improvement Committees to address issues related to medical care coordination in the facility and process improvement initiatives.   

  • Participate in organizing and coordinating the services provided by physicians and other medical professionals. This role must also identify performance expectations for physicians as well as facilitating feedback based on performance and practices.   

  • Work in partnership with the Director of Nursing on developing policies to enhance clinical capabilities, outcomes and resident-centered care.  

  • Promote resident-centered care, including through the formation of advance directives protocols and promoting resident choice in making care decisions. 

  • Act as a liaison between administration and attending physicians.   

  • Partner with the Director of Nursing in relation to patient care services.   

  • Ensure completion of employee’s pre-employment and annual health examination reports.   

  • Communicate with residents and families during care conferences as needed.  

  • Provide education and skill enhancement for the clinical department.  

  • Round to observe care and infection prevention practices, including proactive, with communication to appropriate departments. 

  • Champion quality improvement through data analysis and data trending, identifying quality opportunities and contribute to action plans.  

  • Provide care in the absence of the primary care physician or in the event of an emergency.  

  • Perform other duties as assigned   

How to qualify

Required Minimum Qualifications  

LICENSE AND CERTIFICATION:  

  • A valid California Medical License (MD or DO).*   

  • Member of the California Association of Long-Term Care Medicine 

  • Possession of valid, unrestricted Drug Enforcement Agency registration with the United States Department of Justice. 

  • Certified as a Medical Director by the American Board of Post-Acute and Long-Term Care Medicine.**  

  • Board Certification in a relevant specialty area (e.g. Certificate of added qualifications in Geriatric Medicine from the American Board of Internal Medicine or the American Board of Family Medicine). 

*Applicants possessing a valid license to practice medicine issued from another state within the United States of America may apply, but if selected, the candidate will NOT be appointed/hired until they obtain a valid license to practice medicine issued by the Medical Board of California or the Osteopathic Medical Board of California.

** If not certified by the American Board of Post-Acute and Long-Term Care Medicine, then must initiate the process of certification within 3 months of hire and demonstrate substantial progress every three months thereafter, until certification is achieved. 

EXPERIENCE:  

Requires at least three years of experience as an administrator of a large general teaching hospital or related organization; three years of complex health care administrative experience or an equivalent combination of training and experience.  

The most successful applicants will have: 

  • Masters-level training in an applicable field such as public health, business administration, or equivalent. 

  • Three (3) years of post-residency, managerial experience (of which all must include supervisory experience) in planning, organizing, and evaluating medical services in a hospital, clinic, skilled nursing facility, or related health care setting.  

One (1) year of full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a forty (40) hour work week). Any overtime hours that you work above forty (40) hours per week are not included in the calculation to determine full-time employment.
