The San Francisco Department of Public Health (DPH) is accepting applications in class 2586 Health Worker 2 to work in various locations. Positions may be located at the following locations, but not limited to: Zuckerberg San Francisco General Hospital (ZSFG), Laguna Honda Hospital (LHH), various clinics, Human Services Agency (HSA), Department of Homelessness and Supporting Housing (HOM), Community College District (CCD), San Francisco Sheriff’s Department (SHF), and the Unified School District (USD).

The eligible list resulting from this Class Based Test (CBT) announcement will be used to fill future vacancies in this class for the duration of the eligible list.

Shift: Days, evenings, weekends, and/or holiday shifts may be required, depending on the position.

Role description

General Job Description:
Under supervision, the Health Worker 2 performs a wide variety of paraprofessional duties in a service program. The Health Worker 2 functions as a liaison between community residents and program staff; provides counseling and advice to patients regarding health problems; may supervise Health Worker I and may drive or accompany patients between their homes, hospitals or other social agencies. The essential functions include, but are not limited to the following:

  • Participates, but to a lesser degree than Health Worker I, in the health service training program;
  • Provides information and resources to patients and others regarding health care and other facilities available to them;
  • Assists patients in utilizing services;
  • Makes follow-up contacts when required;
  • Serves as liaison between the professional staff and the community;
  • Assists in gathering and evaluating data concerning the program to which assigned;
  • May perform incidental clerical duties such as keeping records, answering the telephone and arranging client appointments;
  • May transport ambulatory patients between their homes and clinics, hospitals or other social agencies;
  • May transport staff members to meetings with administration approval; • reports malfunctions of vehicle to supervisor;
  • May provide language assistance in contacts with non-English speaking clients; and
  • May pick up and deliver supplies and equipment, including high-security pharmaceutical supplies, laboratory tests and mail.

The 2586 Health Worker 2 performs other related duties as required/assigned.

Working Conditions: Some positions may require employees to work directly with individuals within locked jails or psychiatric facilities; the homeless population and individuals who have or are carriers of infectious/communicable diseases. Some positions may also require exposure to inclement weather conditions and/or heavy lifting.

How to qualify

  1. Experience: One (1) year of verifiable experience within the last five (5) years, performing a combination of at least two (2) of the following duties:
  • serving as a liaison between targeted communities and healthcare agencies; providing culturally appropriate health education/information and outreach to targeted populations;
  • providing referral and follow up services or otherwise coordinating care;
  • providing informal counseling, social support and advocacy to targeted populations;
  • escorting and transporting clients;
  • providing courier/dispatcher functions;
  • performing pre-clinical examinations of vital statistics, such as measuring a patient's weight, height, temperature and blood pressure

Substitution: Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience for minimum qualification #1.

One year of full-time employment is equivalent to 2000 hours. (2000 hours of qualifying work experience is based on a 40-hour work week.) Any overtime hours that you work above forty (40) hours per week are not included in the calculation to determine full-time employment.

Selection Procedures
After application submission, candidates deemed qualified must complete all subsequent steps to advance in this selection process, which includes the following:

Minimum Qualification Supplemental Questionnaire (MQSQ): Candidates will be required to complete a MQSQ as part of the employment application. This MQSQ is designed to obtain specific information regarding an applicant's experience in relation to the Minimum Qualifications (MQ) for this position. The MQSQ will be used to evaluate if the applicant possesses the required minimum qualifications.

Multiple Choice Written Examination (Weight: 100%)
Candidates who meet the minimum qualifications will be invited to participate in a written multiple-choice test designed to measure knowledge, skills and/or abilities in job-related areas, which may include but is not limited to: Ability to establish and maintain effective and cooperative working relationships by demonstrating leadership; Ability to work in a culturally diverse setting with clients who are disabled, physically and chronically ill and/or who may have behavioral health issues; Ability to apply de-escalation and counseling techniques when dealing with patients/clients; Ability to interview patients/clients to evaluate their capabilities, needs, interests and the ability to exercise good judgment when making a patient/client determination; Ability to read, comprehend and interpret information to communicate in a clear and concise manner including the ability to provide educational information about health programs to patients/clients; Ability to type and enter data/information accurately and the ability to coordinate and schedule patient/client appointments; Ability to prepare, organize, code and index records and files; Knowledge of and ability to interpret and apply office policies, laws, regulations (e.g. HIPPA) and procedures in a medical setting.

Candidates must obtain a passing score in order to continue in the selection process and will be placed on the confidential eligible list in rank order according to their written examination score.

Language Proficiency Exam: For positions requiring bilingual fluency, candidates deemed qualified who meet the special language condition must pass a departmentally approved language proficiency exam prior to appointment. Only those eligible candidates who pass the bilingual proficiency test will be considered for the bilingual position. Applicants must indicate on the application form the language(s) in which they claim proficiency.

What else should I know?

Eligible List/Score Report:
A confidential eligible list of applicant names that have passed the civil service examination process will be created and used for certification purposes only. An examination score report will be established, so applicants can view the ranks, final scores, and number of eligible candidates. Applicant information, including names of applicants on the eligible lists, shall not be made public unless required by law. However, an eligible list shall be made available to public inspection, upon request, once the eligible list is exhausted or expired and referrals resolved. The eligible list/score report resulting from this civil service examination process is subject to change after adoption (e.g. as a result of appeals), as directed by the Human Resources Director or the Civil Service Commission.

Certification: The certification rule for the eligible list resulting from this examination will be Rule of Ten Scores. Additional selection processes may be conducted by the hiring department prior to making final hiring decisions

The duration of the eligible list resulting from this examination process will be twelve (12) months and may be extended with the approval of the Human Resources Director.

  1. Licensure/Certification/Registration: Valid licensure/certification/registration as a requirement to perform the job must be kept current throughout length of employment. Failure to demonstrate/show proof of possession of required valid licensure/certification/registration may result in dismissal and/or termination of employment.

  3. Note on Electronic Health Record (EHR): The Department of Public Health (DPH) is implementing a unified Electronic Health Record (EHR) system and DPH employees must demonstrate competency in the use of the system that is appropriate for their classification as a condition of employment.
  4. Note on Personal Protective Equipment (PPE): Some positions in the Department of Public Health will require the use of personal protective equipment (PPE), including but not limited to gloves, gowns, eye and face protection, and face-fitting respirators. The requirement for the use of PPE may come on short or no notice. Facial hair or any condition that interferes with a face-fitting respirator’s seal (i.e. comes between the sealing surfaces of the respirator and the wearer’s bare skin) is not permitted when face-fitting respirators are worn, including during initial or periodic respirator fit-testing. Employees who choose not to shave and do not have either American Disabilities Act (ADA) or Equal Employment Opportunity (EEO) Accommodations do not have the right to alternate work assignments or the option of using a loose-fitting Powered Air Purifying Respirator (PAPR) in place of a FaceFitting Respirator.
  5. Medical Examination/Drug Testing: Prior to appointment, at the Department's expense, applicants may be required to take a tuberculosis (TB) screening test, a medical examination and/or drug test.
