The Mission of the San Francisco Department of Public Health (SFDPH) is to protect and promote the health of all San Franciscans. SFDPH strives to achieve its mission through the work of two main Divisions - the San Francisco Health Network and Population Health. The San Francisco Health Network is the City’s only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers.
The applicant pool resulting from this recruitment may be utilized to fill future temporary exempt vacancies in this class at other locations within the DPH.
Role description
There are various Temporary Exempt (TEX) positions in class 2586 Health Worker 2 at the following locations:
- Community Health Equity and Promotion (CHEP) Branch (1 full-time TEX position. Duration: No more than 13 months)
- Shift: Monday - Friday, 8 AM - 5 PM, some evenings and weekends
- Zuckerberg San Francisco General Hospital, Care Experience (6 as-needed TEX positions. Duration: Up to 6 months)
- Shift 1: Monday and Friday, 8:30 AM - 7 PM (20 hours per week)
- Shift 2: Saturday and Sunday, 6 AM - 5 PM (20 hours per week)
- Shift 3: Saturday and Sunday, 5 PM – 4 AM (20 hours per week - night)
- Shift 4: Tuesday to Saturday, 8:30 AM – 5 PM (40 hours per week)
- Shift 5: Sunday to Thursday, 8:30 AM – 5 PM (40 hours per week)
- Shift 6: Monday – Friday, 6 AM – 2:30 PM (40 hours per week)
- Whole Person Integrated Care (WPIC), Various Shelter and Navigation Centers (2 full-time TEX positions. Duration: Not to exceed 3 years.)
- Shift: Monday - Friday, 8 AM - 5 PM
General Job Description:
Under supervision, the Health Worker 2 performs a wide variety of paraprofessional duties in a service program. The Health Worker 2 functions as a liaison between community residents and program staff; provides counseling and advice to patients regarding health problems; may supervise Health Worker I and may drive or accompany patients between their homes, hospitals or other social agencies. The essential functions include, but not are not limited to the following:
- Participates, but to a lesser degree than Health Worker I, in the health service training program;
- Provides information and resources to patients and others regarding health care and other facilities available to them; assists patients in utilizing services;
- Makes follow-up contacts when required;
- Serves as liaison between the professional staff and the community;
- Assists in gathering and evaluating data concerning the program to which assigned;
- May perform incidental clerical duties such as keeping records, answering the telephone and arranging client appointments;
- May transport ambulatory patients between their homes and clinics, hospitals or other social agencies;
- May transport staff members to meetings with administration approval; reports malfunctions of vehicle to supervisor;
- May provide language assistance in contacts with non-English speaking clients; and
- May pick up and deliver supplies and equipment, including high-security pharmaceutical supplies, laboratory tests and mail.
The 2586 Health Worker 2 performs other related duties as required/assigned.
Working Conditions: Some positions may require employees to work directly with individuals within locked jails or psychiatric facilities; the homeless population and individuals who have or are carriers of infectious/communicable diseases. Some positions may also require exposure to inclement weather conditions and/or heavy lifting.
How to qualify
Experience: One (1) year of verifiable experience within the last five (5) years, performing a combination of at least two (2) of the following duties:
- serving as a liaison between targeted communities and healthcare agencies;
- providing culturally appropriate health education/information and outreach to targeted populations;
- providing referral and follow up services or otherwise coordinating care;
- providing informal counseling, social support and advocacy to targeted populations;
- escorting and transporting clients;
- providing courier/dispatcher functions;
- performing pre-clinical examinations of vital statistics, such as measuring a patient’s weight, height, temperature and blood pressure.
Substitution: Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience.
Applicants must meet the minimum qualification requirement by the final filing date unless otherwise noted.
One-year fulltime employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week). Any overtime hours that you work above 40 hours per week are not included in the calculation to determine fulltime employment.
DESIRABLE QUALIFICATIONS: The stated desirable qualifications may be considered at the end of the selection process when candidates are referred for hiring.
- Experience with crisis intervention, service recovery and/or de-escalation
- Experience working in a hospital setting
- Experience and commitment to working with diverse communities
- Experience directly working with and supporting well-being of populations experiencing health disparities including Latino/a/x, Indigena Black/African American, API and others
- Spanish/English bilingual and biliterate
- Experience conducting outreach and community engagement
- Experience and proficiency with computer organizational and communications tools, including Zoom and MS Office applications including Excel, Outlook, PowerPoint, Teams, and Word
- Effective written and verbal communication skills with individuals, colleagues, and others with a trauma-informed lens
- Comfortable working in various community settings
- Ability to work with others from diverse backgrounds including members of the public, community leaders, CBO, and City staff
- Ability to work independently and proactively with guidance and direction, while maintaining clear, reliable communication with team and partners
- Have a genuine desire to meet the needs of others
- Ability to multi-task and respond to emerging challenges with flexibility
