Appointment Type

Temporary-Exempt (TEX). This position is excluded by the Charter from the competitive civil service examination process and shall serve at the discretion of the Appointing Officer. The duration of the appointment shall not exceed 24 months.

The Department provides assistance and support to homeless and at‐risk youth, adults, and families to prevent imminent episodes of homelessness and end homelessness for people in San Francisco. Services including outreach, homelessness prevention, emergency shelter, drop‐in centers, transitional housing, supportive housing, short‐term rental subsidies, and support services to help people exit homelessness. What We Do

San Francisco is a pioneer in homeless services and a leader in providing supportive housing as a permanent exit from homelessness. The Department of Homelessness and Supportive Housing (HSH), founded in 2016, is a national leader in the movement to end homelessness by developing a coordinated, equity-driven, client-focused system of compassionate services while piloting innovative models, and implementing best practice solutions with measurable results. HSH’s Homelessness Response System (HRS) oversees and implements a system of care that shelters, houses, and serves approximately 14,000 people daily. Major programs include: street outreach and service connection through the Homeless Outreach Team (SFHOT); 3,000-bed shelter system for adults and families including shelters for members of the LGBTQ community; rapid rehousing rental subsidies for families, adults, older adults, and transitional aged youth; the Problem Solving and flexible financial assistance programs; and robust supportive housing programs with nearly 10,000 units which provide permanent housing and services to formerly homeless individuals and families.

Role description

The 2586 Health Worker II with the Department of Homelessness and Supportive Housing (HSH) Housing Services Branch works with a multi-disciplinary team, including Behavioral Health clinicians, Case Managers, Property Management staff, and DPH Nursing staff. Under supervision, the 2586 performs a wide variety of paraprofessional duties within the Enhanced Support and logistics Team (EST) to support permanent supportive housing residents of the Star, Empress, Camelot, Le Nain, Pacific Bay Inn, Windsor Hotel, and Kelly Cullen Community Permanent Supportive Housing (PSH) sites to maintain long-term housing, with independence and stability. The 2586 supports the work of the Support Services team by providing practical support for daily programmatic operational tasks as well as logistical support for interventions determined by the Support Services Clinicians and/or Case Manger's work with residents, to address housing issues and/or health needs.

Essential Duties and Responsibilities:
Under general supervision of the Support Services Administrative Coordinator and Clinical Supervisor, the 2586 Health Worker II provides support for services, outreach, community building, and daily operations. The essential job functions include the following:

  • Supports the work of the Case Management and Behavioral Health Clinical Leads to engage residents into services, care, and activities coordinated by the Support Services team.
  • Provides education and information about community mental health and substance use services, overdose prevention resources, and harm reduction programs. Builds and manages resource databases for the Supportive Services program and ensures information is available and visible to residents and staff.
  • Fills operational gaps in residents connecting to services coordinated by the Case Managers or Clinical Leads, including escorts to health and social service appointments or resources in the community.
  • Participates in group social and educational activities led by both Support Services and Property Management.
  • Supports sites and the EST with procurement of food and supplies for monthly client events at housing sites.
  • Supports the HSH Support Services Administrative Coordinator with daily operational tasks and projects across hotels.
  • Collaborates with members of the inter-disciplinary team, including DPH staff, housing provider staff, and community organizations.

Essential Knowledge, Skills, and Abilities:

  • Knowledge of the ethnic, economic and social factors affecting the residents of the neighborhood served.
  • Ability to provide support to residents in a manner that is both non-discriminatory and non-judgmental.
  • Ability to effectively communicate with residents and other staff members. 
  • Demonstrated problem-solving skills and strong inter-personal and relationship-building skills.

How to qualify

Experience:
One (1) year of verifiable experience within the last five (5) years, performing a combination of at least two (2) of the following duties: Serving as a liaison between targeted communities and healthcare agencies; providing culturally appropriate health education/information and outreach to targeted populations; providing referral and follow up services or otherwise coordinating care; providing informal counseling, social support and advocacy to targeted populations; escorting and transporting clients; providing courier/dispatcher functions; performing pre-clinical examinations of vital statistics, such as measuring a patient’s weight, height, temperature and blood pressure.

License and Certification:
Possession of a valid California Driver's License 

Substitution:
Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience.

Desirable Qualifications:

  • Bilingual or multi-lingual language capacity
  • Ability to work independently
  • Experience facilitating or assisting with social and educational groups/activities
  • Experience working as a "peer" in social service, community, housing, or health settings
  • Comfort and familiarity working within a harm reduction model
  • Basic working knowledge of Microsoft Outlook, Word, Forms, and Excel
  • One or more years experience working with people with multiple, complex health and social needs in a residential setting or inpatient program
  • One or more years experience working with Property Management and/or housing services
  • Knowledge of primary care, psychiatric services and social services, and substance abuse treatment programs and resources available in San Francisco
