Role description

Under general supervision, the 2920 Medical Social Worker performs duties related to providing access to services for Older Adults and Adults with Disabilities through DAS Intake Call Center/Helpline and DAS Benefits and Resources Hub.  The 2920 Medical Social Worker evaluates social, emotional and physical needs of clients and provides available service options to address their needs. The 2920 Medical Social Worker also participates in DAS outreach to the community.

ESSENTIAL DUTIES AND FUNCTIONS

According to Civil Service Commission Rule 109, the duties specified below are representative of the range of duties assigned to this job class and are not intended to be an inclusive list; may include additional duties as assigned.

  1. Perform comprehensive integrated intake assessments and consultation to older adults and adults with disabilities via telephone, face-to-face and /or online interactions. Navigates caller to appropriate referrals/intakes/applications to the following county programs: Information and Referrals (I&R) for resources within San Francisco; Adult Protective Services; In-Home Support Services; Community Living Fund; Home Delivered Meals; Case Management; Options Counseling, and other programs as necessary.
  2. Conduct daily follow-up calls for messages or emails sent to DAS Intake Unit from individuals and referents seeking Information and Referral services, IHSS, Options Counseling, CM, HDM and CLF; conduct monthly follow-up calls to clients who are on the waitlist for CM, CLF and HDM so as to monitor any change in client's service needs.
  3. Support agency's mission of 'no wrong door' by providing Information and Referrals to clients at the DAS Resources and Benefits HUB service counter. Contribute to the unit's general goal of providing clientcentered services by assessing client's overall service needs and making cross referrals to other DAS programs as needed.
  4. Comply with agency policy and adhere to the HIPPA regulations; conduct mandatory reporting activities by completing reports (CPS, APS, Ombudsman for elder abuse) in specific timeframe; have thorough understanding of the DAS Employee Intake Procedure and complete assignment satisfactorily and timely with minimal supervision.
  5. Provides outreach and informational presentations to community partners and clients. Participates in community work groups and committees involving older people and adults with disabilities when the opportunity arises.
  6. Demonstrate competence in using computer systems and software so as to perform daily assignment to enhance work productivity. These include CMIPS II, LEAPS, SF DAS GetCare, Blackberry Application with work cell phone, Microsoft Outlook, MS Teams, Microsoft Word and Excel.
  7. Attend training and other continuous education opportunities to strengthen professional knowledge and actualize individual skill level; come prepared for monthly supervision and show interest in gaining a deeper understanding of client motivation and actions.
  8. Monitor APS reports and other online referrals when assigned by supervisors or by iTasks; observe the timeframe in acting on the cases.
  9. Perform disaster service as needed.

How to qualify

  1. Education: Possession of a Master's Degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE);

AND

  1. Experience: Six (6) months (1000 hours) of social work experience in a health care setting. This experience may be gained from supervised field placement during the Master's in Social Work (MSW) program or from post-graduate social work experience.

 

DESIRABLE QUALIFICATIONS AND SKILLS

The stated desirable qualifications and skills may be used to identify job finalists at the end of the selection process when candidates are referred for hiring.

  • Social Work experience and knowledge working with Older Adults and/or Adults with Disabilities
  • Knowledge of resources and services for San Francisco residents
  • Excellent verbal and written communications skills
  • Ability to manage multiple tasks/assignments

Selection Procedures:

The selection process will include evaluation of applications in relation to minimum requirements and assessment of candidates’ job-related knowledge, skills and abilities in an oral interview, which may include a written or performance exercise.

Depending on the number of applicants, the Department may establish and implement additional screening mechanisms to comparatively evaluate qualifications of candidates. If this becomes necessary, only those applicants whose qualifications most closely meet the needs of the Department will be invited to continue in the selection process. Applicants meeting the minimum requirements are not guaranteed advancement in the selection process.
