The Mission of the San Francisco Department of Public Health (SFDPH) is to protect and promote the health of all San Franciscans. SFDPH strives to achieve its mission through the work of two main Divisions - the San Francisco Health Network and Population Health. The San Francisco Health Network is the City’s only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers.

Laguna Honda Hospital and Rehabilitation Center is looking for an As-Needed candidate with experience conducting eligibility, registration, admissions, transfers, TAR submissions (using EPIC a plus), and referral intake in a SNF/Acute hospital, Health at Home, and Emergency setting.  The applicant pool resulting from this recruitment may be utilized to fill future and current vacancies in this class at other locations within the DPH.

Role description

Under general supervision, the Hospital Eligibility Worker performs various technical duties in the screening, determination of patient eligibility, and enrollment for medical coverage under the terms of various private and public health care and financial assistance programs.

The essential job functions include the following:

  • Reviews and manages referrals
  • Acts as a liaison
  • Maintains referral records and assigns patients to providers
  • Schedules patients and collects insurance information
  • Secures and evaluates outside medical records
  • Prioritizes patient referrals based on the service requested
  • Communicates with patients, physicians, and hospital staff
  • Obtains and documents insurance authorizations
  • Prioritizes registration and maintains reception area, assists patients with outpatient check-in
  • Assumes front-end office responsibilities, which include answering in-coming referral inquiry calls, responding to patient calls, and documenting information
  • Performs EPIC Transfer center-specific functions, schedules outpatient appointments received, updates records submits required reports
  • Identifies, assists, enrolls, and applies clients for entitlement programs and updates patient information for families.
  • Assists patients in interpreting clinic bills, consulting with the billing office on the patient's behalf, and making direct referrals to the billing office when needed
  • Composes and prepares correspondence to patients, references, and State agencies
  • Gathers statistical data and prepare reports as required
  • Answer questions and provide information to patients and the general public regarding the assigned program area
  • Refers incoming callers and patients to appropriate case management and nursing staff

The 2903 Hospital Eligibility Worker performs related duties as assigned/required.

Shifts: M-F (varied As-Needed), weekends, and/or holiday shifts may be required, depending on position. The hospitals operate on a 24-hour, 7 day per week basis and some Department of Public Health centers/clinics are open evenings, nights, weekends, and/or holidays. 

How to qualify

Minimum Qualifications:

1. Two (2) years of direct public contact clerical work experience which must have included ALL of the following:

  • use of computer applications to input, retrieve, and analyze information;
  • working with a diverse client population;
  • performing responsibilities involving interviewing to acquire detailed personal or confidential information;
  • interpreting and applying rules, regulations, and policies;
  • creating and processing documents related to the financial or personal histories of clients; and
  • assisting clients with the completion of forms or correspondence;

                OR

2. One (1) year of experience determining eligibility for health and/or social services programs, loans, financial assistance, unemployment, or veterans benefits

                OR

3. Sixty (60) semester units or ninety (90) quarter units from an accredited college or university. (Please specify in your application how many units you have completed and/or if you have completed your degree)

Substitution: Thirty (30) semester units or forty-five (45) quarter units from an accredited college or university may substitute for one (1) year of work experience under qualification pattern number one (1) above. (Please specify in your application how many units you have completed and/or if you have completed your degree)

One year of full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week). Any overtime hours that you work above 40 hours per week are not included in the calculation to determine full-time employment.
