The Mission of the San Francisco Department of Public Health (SFDPH) is to protect and promote the health of all San Franciscans. SFDPH strives to achieve its mission through the work of multiple divisions - the San Francisco Health Network, Population Health, Behavioral Health Services, and Administration. The San Francisco Health Network is the City’s only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers. The Population Health Division (PHD) provides core public health services for the City and County of San Francisco: health protection, health promotion, disease and injury prevention, and disaster preparedness and response. Behavioral Health Services operates in conjunction with SFHN and provides a range of mental health and substance use treatment services.

Role description

Under the general direction of the Supervising Physician, nurse practitioners are an integral team member, providing advanced practice nurse clinical care and clinical instruction. Nurse practitioners staffing this telemedicine line will engage and evaluate patients interested in starting buprenorphine, prescribe buprenorphine as appropriate and generally ensure the provision of patient-centered, evidence-based treatment. 

  • Act as “First Call” provider on BEAM telemedicine line, answering calls immediately, providing excellent client-centered care, and ensuring “Second Call” provider availability and messages are responded to in a timely manner. 

  • Perform medical history for intake evaluation via telemedicine Assess and diagnose opioid use disorder (OUD) and counsel on different Medication for Opioid Use Disorder (MOUD) treatment options. 

  • Assess treatment needs and, when appropriate, prescribe buprenorphine to individuals seeking treatment for OUD. 

  • Provide crisis intervention including developing a treatment plan or facilitating referral to an appropriate agency. 

  • Provide focused therapy and education to patients, including but not limited to harm reduction, relapse prevention, and motivational interviewing. 

  • Document clinical encounters and use electronic prescribing in EPIC electronic medical record system. 

  • Perform other clinical duties as needed. 

  • Attend and sufficiently complete a BHS-led training on the 24/7 Buprenorphine Telemedicine Line and ongoing training as required. 

  • Attend and participate in meetings required, including but not limited to weekly 24/7 buprenorphine line program meetings. 

  • Work with Behavioral Health Access Line and BEAM navigation staff to ensure patients are referred appropriately and to optimize transitions in care. 

The 2328 Nurse Practitioner (BEAM) also performs other related duties as required.

How to qualify

REQUIRED MINIMUM QUALIFICATIONS 

  1. LICENSE: Possession of a valid California Registered Nurse (RN) license issued by the California Board of Registered Nursing; AND

  2. LICENSE: Possession of a valid California Nurse Practitioner (NP) license issued by the California Board of Registered Nursing; AND
     
  3. LICENSE: Possession of a current medication furnishing license issued by the California Board of Registered Nursing; AND
     
  4. CERTIFICATION: Possession of national board certification, or eligibility, as a Nurse Practitioner issued by a national certifying body or organization; AND
     
  5. CERTIFICATION: Possession of a Drug Enforcement Agency (DEA) number to furnish controlled substances
