The Mission of the San Francisco Department of Public Health (SFDPH) is to protect and promote the health of all San Franciscans. SFDPH strives to achieve its mission through the work of multiple divisions - the San Francisco Health Network, Population Health, Behavioral Health Services, and Administration. The San Francisco Health Network is the City’s only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers. The Population Health Division (PHD) provides core public health services for the City and County of San Francisco: health protection, health promotion, disease and injury prevention, and disaster preparedness and response. Behavioral Health Services operates in conjunction with SFHN and provides a range of mental health and substance use treatment services.

Role description

The Director of Care Coordination provides vision and leadership for key hospital departments and clinic functions, including Utilization Management and Medical Social Services. This role involves developing strategic goals, operational objectives, and policies aligned with the mission of Zuckerberg San Francisco General Hospital (ZSFG) and the Department of Public Health. Externally, the Director handles policy directives and service delivery on complex issues across the Department of Public Health, the San Francisco Health Network (SFHN), and public welfare.

The Director of Care Coordination (0942 Manager VII) performs the following essential job functions:

  • Provides vision and leadership for Utilization Management and Medical Social Services, aligning departmental goals with the values, mission, and strategic plans of ZSFG and SFHN.
  • Guides department and service managers in setting strategic goals, operational objectives, and policies that reflect the values and missions of ZSFG and SFHN.
  • Utilizes Joint Commission Leadership Standards and the Centers for Medicare and Medicaid Services Conditions of Participation to plan, direct, and coordinate services in response to the need of the hospital and its patients, staff, and community. Ensures compliance with all regulatory elements of performance related to Utilization Management and Social Services. Works with various hospital departments, including Regulatory, Nursing, Environment of Care, and Risk Management to develop and implement corrective plans as needed.
  • Directs and oversees the planning and development of activities with other hospital department personnel to enhance patient care. Maintains high quality of in-service training and adequacy of training materials to ensure optimal performance.
  • Guides the development and maintenance of service delivery standards, quality measures, productivity standards, customer satisfaction standards, and labor relations. Monitors departmental performance and implements improvement plans.
  • Participates in hospital committees, including the Executive Staff Committee, Quality Council, Senior Leadership, Administrative Operations, Accreditation, Licensing and Certification Committee, and Nursing Executive Committee.
  • Provides leadership by identifying and analyzing problems, implementing appropriate actions, and evaluating decision effectiveness. Serves on committees to assist in policy, procedure, and program planning and execution, and participates in budgetary and quality assurance processes.

The Director of Care Coordination (0942 Manager VII) may perform other duties as assigned/required.

How to qualify

Required Minimum Qualifications

  1. Education: Bachelor's degree from an accredited college or university; AND
  2. Experience: Five (5) years of experience managing a nursing, social work, or utilization department, of which all must include supervisory experience.

Education Substitution: Additional qualifying experience may substitute for the degree requirement on a year-for-year basis for up to two (2) years. One year (2,000 hours) of qualifying experience will be considered equivalent to 30 semester units/45 quarter units.

One year full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week).

Desirable Qualifications: The stated desirable qualifications may be considered at the end of the selection process when candidates are referred for hiring:

  • Possession of a Master’s degree from an accredited college or university
  • Experience working in an acute care hospital environment
  • Experience as a clinical or health care professional
  • Professional certification related to care coordination or case management

Selection Procedures:

Supplemental Questionnaire (SQ) Examination (Weight: 100%)
Candidates that meet the minimum qualifications will be invited to participate in a Supplemental Questionnaire (SQ) exam that is designed to measure the knowledge, skills, and abilities in job related areas which may include but not be limited to: ability to develop effective and productive working relationships with hospital senior leadership, department managers, and external entities; ability to develop and implement division goals, objectives, policies, and priorities, and to determine service levels and resource allocation; knowledge of regulatory requirements governing an acute care facility, e.g. regulations and codes from The Joint Commission, Title XXII, and other federal and state governing and licensing bodies; knowledge of nursing/patient care theory to assess patient care needs.

Candidates must achieve a passing score on the Supplemental Questionnaire exam in order to continue in the selection process and will be placed on the confidential eligible list in rank order according to their final score.

The certification rule for the eligible list resulting from this examination will be the Rule of the List.

Eligible List/Score Report:

The duration of the eligible list resulting from this examination process will be six (6) months and may be extended with the approval of the Human Resources Director.
