The Mission of the San Francisco Department of Public Health (SFDPH) is to protect and promote the health of all San Franciscans. SFDPH strives to achieve its mission through the work of multiple divisions - the San Francisco Health Network, Population Health, Behavioral Health Services, and Administration. The San Francisco Health Network is the City’s only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers. The Population Health Division (PHD) provides core public health services for the City and County of San Francisco: health protection, health promotion, disease and injury prevention, and disaster preparedness and response. Behavioral Health Services operates in conjunction with SFHN and provides a range of mental health and substance use treatment services.  

Role description

Under general direction of the Chief Financial Officer, the Revenue Cycle Director is responsible for overseeing all reimbursement programs in multiple divisions within the San Francisco Health Network (SFHN), including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, primary care clinics, ambulatory care services, Behavioral Health Services (BHS), and Population Health Division (PHD). Overall, this position oversees the claiming and projection for all of SFDPH’s operating revenue of approximately $2 billion. The Revenue Cycle Director provides vision and leadership in managing complex state and federal healthcare safety net financing programs in alignment with the mission of SFDPH and SFHN and ensuring alignment with expected reimbursement rates and contracted rates awarded to providers. This position works closely with SFHN, BHS and PHD executive leadership to develop revenue and reimbursement strategies; oversees compliance with various financial reporting requirements and financial audits; designs and oversees complex financial analyses of costs and revenue to inform policy and operational changes; plays a leadership role in the optimization and management of the department’s EPIC electronic health record, including supervising the cash management and accounting functions for the EPIC single billing office; represents SFDPH in working with the Mayor’s Budget Office and Controller’s Office on financial planning and budgeting for health revenue; and serves on key external decision making bodies influencing State policies and laws affecting reimbursement including the San Francisco Health Plan and the California Association of Public Hospitals.

The Revenue Cycle Director performs the following essential job functions:

  • Plans, monitors, evaluates, supervises, and coordinates the activities of multiple functions related to revenue and reimbursement for SFDPH including the Revenue/Reimbursement Department, EPIC Single Billing Office Cash Management, and associated accounting functions. 
  • Serves as principal advisor on revenue projections and variance analysis to SFDPH executive, clinical, finance, and operational leaders on issues affecting revenue and reimbursement; recommends policy and operational changes to increase reimbursement levels.
  • Represents the department in interactions with senior leadership in the Controller’s Office, Mayor’s Budget Office, State agencies, and other key stakeholders on revenue and reimbursement issues affecting the City’s Five-Year Financial Plan, annual budgets, and citywide quarterly financial reporting.
  • Plays a leadership role in executive and policy making committees affecting the implementation, optimization, and operation of the EPIC electronic health records system; develops strategies to improve data collection and analysis to increase revenue.
  • Plans and oversees highly complex cost and revenue analyses of programs affecting multiple service areas within the department, requiring coordination of multiple branches within SFDPH Finance.
  • Plans and supervises compliance with various financial reporting requirements and participation in audits for Medicare, Medi-Cal, the Office of Statewide Health Planning and Development (OSHPD), and other agencies.
  • Oversees reporting and reimbursement activities associated with specialized State and Federal healthcare safety net financing programs, such as the CalAIM, Global Payment Program, Public Hospital Redesign and Incentives in Medi-Cal (PRIME) program, Quality Incentive Program, Enhanced Payment Program, and other supplemental funding programs. 
  • Oversees new or changes to BHS lines of business, including transition to EPIC and other changes within the County Medi-Cal Mental Health (MH) and Substance Use Disorder (SUD) Managed Care Plans; Performs planning, design, and implementation of encounter data reporting, billing, and claims processing.
  • Supervises the cash management and general accounting functions related to EPIC, SFHN’s enterprise electronic health record and billing system. Oversees integration and reconciliation between EPIC and the City’s PeopleSoft financial system, collection and payment posting of patient liabilities, and reconciliation of bank records to the EPIC system.
  • Represents SFDPH on policy and technical decision-making bodies affecting state and federal revenue, including executive level representation with the California Association of Public Hospital Hospitals and Health Systems; advises SFDPH and citywide leaders on policy and advocacy strategies to increase reimbursement.
  • The Revenue Cycle Director may perform other duties as assigned/required.

How to qualify

Required Minimum Qualifications (all applicants must verify that they meet all elements below):

Education: Possession of a Bachelor’s degree from an accredited college or university, AND

Experience: Four (4) years of managerial experience in revenue and reimbursement management, optimization, and accounting in a hospital or healthcare system. All of which must include supervising professionals.

Education Substitution: Additional qualifying experience as described above may be substituted for the required degree on a year-for-year basis up to a maximum of two (2) years. One year is equivalent to thirty (30) semester or forty-five (45) quarter units.

Note: One-year fulltime employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week).

Desirable Qualifications (the stated desirable qualifications may be considered at the end of the selection process when candidates are referred for hiring.):

  • Master’s degree in Business Administration, Accounting, Finance, Healthcare Administration, Public Policy, Economics, or a closely related field.
  • Qualifying experience obtained in a large public or safety net hospital.
