The San Francisco Department of Public Health (SFDPH), Gender Health SF (GHSF) is accepting applications for one (1) full time temporary provisional position in class 2587 Health Worker 3. The Gender Health Navigator will function as a lead peer patient-access navigator responsible for providing transgender and nonbinary culturally congruent peer navigation services, program leadership and support.  The lead peer patient-access navigator will maintain a caseload of patients at-risk for and living with HIV to navigate for gender-affirming surgeries and provide peer navigation support to trans and nonbinary peer staff under the guidance and supervision of the behavioral health Clinical Director. 

GHSF is a dynamic and unique Transgender and Gender Diverse (TGD) culturally congruent program and the first of its kind in the United States.  Our mission is to provide access to gender-affirming surgeries and related education and preparation services to eligible uninsured TGD residents of San Francisco, contributing to improved quality of life and returning consumers to medical and mental health care.  SFDPH currently offers a range of health services to TGD residents, such as primary care, prevention, behavioral health, hormone therapy, specialty, and inpatient care. GHSF is located on the Zuckerberg San Francisco General Hospital (ZSFGH) campus and is part of the Behavioral Health Services (BHS) of SFDPH.

Schedule: Monday - Friday, 8 AM - 5 PM

Role description

Under the close supervision and guidance of GHSF’s behavioral health Clinical Director, the 2587 Health Worker 3, Gender Health Navigator performs a wide variety of well-defined duties and works with professional staff in extending effective culturally congruent services to GHSF patients; plans, develops, and follows through on all contacts and cases designed to aid and encourage members of the community to utilize Health Department services. The essential duties include, but are not limited to the following: 

  • Provide peer patient-access navigation services to TGD patients, including monolingual and bilingual Spanish-speaking individuals;
  • Perform case management and peer navigation activities that support HIV + and HIV- or unknown status clients to maintain optimal health outcomes with a focus on obtaining and sustaining HIV viral suppression in HIV+ clients, HIV testing for those of unknown HIV status and orientation, referral, and support for initiation and adherence to PrEP for those who are HIV negative;
  • Advocate for patient needs within the systems of care.
  • Offer patient education & preparation support to TGD individuals seeking gender-affirming surgeries;
  • Reinforce TGD informed consent models through pre-surgical information, patient education, preparation, and planning for surgical procedures;
  • Provide patient accompaniment and supports to TGD individuals experiencing psychosocial barriers to care;
  • Provide peer mentorship support for entry-level peer patient navigators and participate in ongoing peer-to-professional workforce development training and in-services for peer staff and people at-risk and living with HIV;
  • Co-facilitate weekly Navigator Meeting;
  • Co-lead in developing peer patient wellness and health education activities that support optimal surgical outcomes for TGD individuals;
  • Coordinate and deliver presentations to patients and health professionals on gender-affirming surgeries and improving health outcomes and care for TGD individuals;
  • Actively demonstrate and encourage team building and clear communication among navigators, other GHSF staff, and program leadership;
  • Provide resource linkage related to pre-surgical preparation and aftercare, including care coordination with referring providers and linkage and engagement to primary care, HIV prevention, and/or HIV care services;
  • Maintain patient information and record-keeping utilizing the program’s electronic database and health records, and other data and evaluation tracking systems that comply with program needs, regulations, and standards of care;
  • Maintain professional and collegial relationships with primary care providers, mental health professionals, and TGD community members;
  • Support program evaluation activities and efforts; and
  • Represent Gender Health SF at meetings through DPH’s systems of care and in the community.

The 2587 Health Worker 3 performs other related duties as assigned/required.

How to qualify

Experience:

  1. Two (2) years of verifiable experience within the last five (5) years, working with a culturally diverse population performing a combination of at least two (2) of the following duties:
  • serving as a liaison between targeted communities and healthcare agencies;
  • providing culturally appropriate health education/information and outreach to targeted populations;
  • providing referral and follow up services or otherwise coordinating care;
  • providing informal counseling, social support and advocacy to targeted populations;
  • escorting and transporting clients;
  • providing courier /dispatcher functions;
  • performing pre-clinical examinations of vital statistics, such as measuring a patient’s weight, height, temperature and blood pressure; AND

    SPECIAL CONDITIONS

      2.Twelve (12) months of experience working with the transgender, transsexual, or gender non-binary population in an administrative capacity or providing direct service in an educational, health, community-based or other program serving the medical, emotional health, socioeconomic or other needs of transgender, transsexual, or gender non-binary communities

 Substitution: Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience for minimum qualification #1.

The above minimum qualifications reflect special conditions associated with the position(s) to be filled. They may differ from the standard minimum qualifications associated with this class code.

One year of full-time employment is equivalent to 2000 hours. (2000 hours of qualifying work experience is based on a 40-hour work week.) Any overtime hours that you work above forty (40) hours per week are not included in the calculation to determine full-time employment.

Desirable Qualifications: The stated desirable qualifications may be considered at the end of the selection process when candidates are referred for hiring.

  • Knowledgeable of gender-affirming surgeries.
  • Experience with providing healthcare, paraprofessional, and peer patient navigation support to TGD individuals.
  • Familiarity with public health populations especially marginalized, poor, under-resourced TGD individuals. 
  • Demonstrated experience with TGD communities and social determinants of health that impact the lives and health outcomes of TGD people. 
  • Knowledgeable of issues affecting TGD immigrants and immigration status.
  • Experience in developing and delivering presentations to enhance patient education and in support of the informed consent model and frameworks.   
  • Experience with using electronic health records such as EPIC, CareLink, and Access database.
  • Knowledgeable of healthcare utility management including San Francisco Health Plan, Healthy San Francisco, Department of Care Coordination access and utilization.
  • Knowledge of the medical, mental health, social determinates of health, and cultural needs of the transgender and gender diverse communities. 
  • Familiarity with communities of color and desire to develop cultural humility practices.
  • Desire to work sensitively with Black Indigenous People of Color and trans and gender diverse populations.
  • Demonstrated ability to provide culturally appropriate health education/information and outreach to targeted population.  
  • Ability to work sensitively with diverse cultural, racial, and gender groups including immigrant populations.
  • Commitment to public health. 
  • Ability to work cooperatively and effectively in a multidisciplinary team.
