The Mission of the San Francisco Department of Public Health (SFDPH) is to protect and promote the health of all San Franciscans. SFDPH strives to achieve its mission through the work of multiple divisions - the San Francisco Health Network, Population Health, Behavioral Health Services, and Administration. The San Francisco Health Network is the City’s only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers. The Population Health Division (PHD) provides core public health services for the City and County of San Francisco: health protection, health promotion, disease and injury prevention, and disaster preparedness and response. Behavioral Health Services operates in conjunction with SFHN and provides a range of mental health and substance use treatment services.

The eligible list resulting from this recruitment may be utilized to fill future and current vacancies in this class at locations within DPH for the duration of the eligible list. 

Role description

Under immediate supervision of licensed professional staff, the Home Health Aide assists clients with activities of daily living including personal hygiene, ambulation, prescribed exercises, preparing meals including therapeutic diets, and keeping client’s environment organized and neat, and performing related duties.

The Home Health Aide performs the following essential duties:

  • Assists clients with personal hygiene including skin and mouth cleansing, hair care, and bathing; assists clients in and out of bed, and with ambulation.
  • Assists clients with prescribed exercises as instructed by professional staff.
  • Assists in keeping client environment neat, safe and organized; changes bed linens as needed; prepares meals, including therapeutic diets; assists clients with eating.
  • Takes client blood pressure and weighs client as assigned.
  • Participates in developing and implementing client care plans according to Home Health Agency policy; communicates client needs and information relating to client care plan to professional staff.
  • Adheres to Home Health Agency administrative and clinical policies and procedures; implements infection control and safety policies and procedures according to established guidelines; maintains confidentiality of client information; provides care in compliance with Home Health Agency, State and Federal regulations, and documents care as assigned.
  • Participates in weekly case conferences related to needs assessments of clients; assists in promoting health education for clients and client families.
  • The Home Health Aide may perform other duties as assigned/required.

Minimum Qualifications

CERTIFICATION: Possession of a Home Health Aide Certificate issued by the State of California

Some positions may require the following:

  1. Bilingual fluency in a variety of languages depending upon the department's bilingual needs. Only those eligible candidates who pass the bilingual proficiency test will be considered for bilingual positions. Applicants must indicate on the application form the language(s) in which they claim proficiency.
  2. Possession of a valid Cardiopulmonary Resuscitation (CPR) Certificate issued by the American Heart Association. Applicants must indicate on the application form the certifications that they possess.

Selection Procedures:
After application submission, candidates deemed qualified must complete all subsequent steps to advance in this selection process, which includes the following:

Training and Experience Evaluation/Supplemental Questionnaire (SQ) (Weight: 100%)
The Training & Experience Evaluation SQ can be completed now and will also be sent to those qualified candidates who demonstrate to meet the minimum qualifications. The SQ is designed to measure knowledge, skills and/or abilities in job related areas that have been identified as critical for satisfactory performance in this position. All relevant experience, education and/or training must be on the application in order to be reviewed in the scoring process. Candidate responses on the SQ cannot be changed once submitted.

Candidates must achieve a passing score on the Training and Experience Evaluation in order to continue in the selection process and will be placed on the confidential eligible list in rank order according to their final score.

Additional selection processes may be conducted by the hiring department prior to making final hiring decisions.

Candidate names will remain on the list for a maximum period of 12 months. Candidates that are not selected and expire off the list may re-apply.
