The Mission of the San Francisco Department of Public Health (SFDPH) is to protect and promote the health of all San Franciscans. SFDPH strives to achieve its mission through the work of two main Divisions - the San Francisco Health Network and Population Health.

The San Francisco Health Network is the City's only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers.

Jail Health Services
San Francisco Department of Public Health (DPH) Jail Health Services (JHS), a section of the San Francisco Health Network’s Ambulatory Care Division, provides comprehensive patient care and coordination for people who are incarcerated in the San Francisco City and County Jail system. JHS is responsible for the care of 1,200-1,300 individuals on any given day, 12,000 individuals over the course of the year. Our team of seven Nurse Practitioners and two Medical Doctors provide 14,000 patient visits a year. JHS provides a unique blend of acute management of urgent issues such as substance withdrawal as well as full scope primary care including, urgent care, chronic disease management, specialty referral to Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG) and prevention on an individual and population level. Our patients are 85% male, 36% Black, 31% White, 77% under 45 and 1% over 65. Seventy percent (70%) stay in jail less than one month at a time, and 28% have more than one incarceration annually.

JHS along with the San Francisco Health Network is in the midst of a journey towards higher efficiency, higher quality, safe and patient centered healthcare delivery.

Role description

The Jail Health Services Nurse Practitioner (NP) acts as a clinical leader of healthcare staff and an advocate for patients. Utilizing standardized procedures, the Jail Health Services NP functions within an organized system for the delivery of health care in association with other members of the multidisciplinary health care team and performs specialized Nurse Practitioner duties. The Jail Health Services NP provides comprehensive, coordinated and high quality care to all incarcerated individuals in the San Francisco County Jails. On a daily basis, this position will work closely with other nurse practitioners, physicians, nurses, licensed and unlicensed behavioral health clinicians and other health workers. Communication and collaboration with Sheriff’s Department staff is critical.

The essential functions include, but are not limited to:

  • Provides full scope primary care: Collects medical histories, reviews outside records, performs physical examinations, interprets and evaluates diagnostic tests, and establishes treatment plans and reassessments; counsels and educates patients regarding both preventive care and chronic diseases; addresses substance abuse; makes appropriate mental health and social service referrals; refers patients to further sources of medical care when appropriate (e.g., ZSFG specialty clinics and Emergency Department); appropriately documents patient information in the electronic medical record;
  • Participates in clinician staff meetings/conferences;
  • Participates in and leads clinical quality improvement efforts;
  • May supervise the work of other healthcare staff; and
  • Participates in population level public health activities, e.g., STD screening, vaccination, etc.

The Nurse Practitioner (Jail Health Services) performs other related duties as assigned/required.

How to qualify

REQUIRED MINIMUM QUALIFICATIONS

1.  SPECIALTY AREA REQUIREMENTS: (Jail Health Services)

a. Specialization as a Family NP, Adult NP, Adult Gerontology NP or Women's Health NP; AND

b. The ability to obtain Security Clearance from the San Francisco Sheriff's Department; AND

2.   LICENSE: Possession of a valid California Registered Nurse (RN) license issued by the California Board of Registered Nursing; AND

3.   LICENSE: Possession of a valid California Nurse Practitioner (NP) license issued by the California Board of Registered Nursing; AND

4.   LICENSE: Possession of a current medication furnishing license issued by the California Board of Registered Nursing; AND

5.   EDUCATION: Possession of a Master’s Degree in Nursing (MSN) from an accredited college or university; AND

6.   CERTIFICATION: Possession of national board certification, or eligibility, as a Nurse Practitioner issued by one of the following recognized national certifying body or organization:

a.  American Academy of Nurse Practitioners (AANP)
b.  American Nurses Association – American Nurses Credentialing Center (ANCC)
c.  Pediatric Nursing Certification Board
d.  National Certification Corporation (NCC) for the Women’s Health Care and Neonatal Nursing Specialties
e.  American Association of Critical Care Nurses
