Role description

Location: Positions may be located at the following locations, but not limited to:  Zuckerberg San Francisco General Hospital (ZSFG), Laguna Honda Hospital (LHH), various clinics within DPH, the Unified School District (USD), and the Department of Homelessness and Supportive Housing (HOM).

Shift: Varies depending on position. Days, evenings, nights, weekends, and/or holiday shifts may be required.

The eligible list resulting from this Class Based Test (CBT) announcement will be used to fill future vacancies in this class at various locations.

General Job Description: Under general supervision, the 2588 Health Worker 4, supervises, coordinates, and instructs a staff of paraprofessional workers in a service program; participates in organizing and directing in-service training activities for Health Workers; provides administrative supervision for Health Workers and assists in planning for their most effective use in the assigned program; and provides community oriented direct and indirect services. The essential functions include:

  • Plans, directs, and coordinates the educational, recreational, therapeutic and work projects of the program participants as directed by professional­ staff;
  • Supervises and trains lower classification of paraprofessional personnel in the skills required for successful performance of their assigned duties;
  • Assists in preparation of in-service training materials;
  • Provides liaison with Team Leaders regarding function and performance of Health Workers 1, 2, and 3;
  • Meets with representative of the local community and professional and social agencies to inform them of the scope of available treatment programs and to elicit their cooperation and support;
  • Provides community education services;
  • Assists in planning and execution of programs and operations, research projects, and reports, as directed;
  • Evaluates data and suggests programs in the preventive areas of social and health adjustments;
  • May interview and screen program clients, identifying general client condition;
  • Performs crisis intervention activities under professional direction;
  • Provides broad social counseling activities and assists in group therapy sessions;
  • When assigned to a specialized activity program, is responsible for supervising the program on specified wards or units and/or assisting the director of the program; and
  • May supervise a specific health or rehabilitation program.

The 2588 Health Worker IV performs other duties as assigned/required.

Working Conditions: Some positions may require employees to work directly with individuals within locked jails or psychiatric facilities; the homeless population and individuals who have or are carriers of infectious/communicable diseases. Some positions may also require exposure to inclement weather conditions and/or heavy lifting.

How to qualify

Experience: Three (3) years of verifiable experience within the last five (5) years, performing a combination of at least two (2) of the following duties:

  • serving as a liaison between targeted communities and healthcare agencies;
  • providing culturally appropriate health education /information and outreach to targeted populations;
  • providing referral and follow up services or otherwise coordinating care;
  • providing informal counseling, social support and advocacy to targeted populations;
  • escorting and transporting clients;
  • providing courier/dispatcher functions;
  • performing pre-clinical examinations of vital statistics, such as measuring a patient’s weight, height, temperature and blood pressure.

Substitution: Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience.

Applicants must meet the minimum qualification requirement by the final filing date unless otherwise noted. 

One-year fulltime employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week). Any overtime hours that you work above 40 hours per week are not included in the calculation to determine fulltime employment.

  • Activity Therapist work experience

Selection Procedures
After application submission, candidates deemed qualified must complete all subsequent steps to advance in this selection process, which includes the following.

Minimum Qualification Supplemental Questionnaire (MQSQ):Candidates will be required to complete an MQSQ as part of the employment application. This MQSQ is designed to obtain specific information regarding an applicant's experience in relation to the Minimum Qualifications (MQ) for this position. The MQSQ will be used to evaluate if the applicant possesses the required minimum qualifications.

Written Multiple Choice Exam (Weight: 100%)
Candidates that meet the minimum qualifications will be invited to participate in the Multiple Choice Exam exam that is designed to measure the knowledge, skills, and abilities in job related areas which may include but not be limited to: Ability to establish and maintain effective relationships with others of various ethnic, cultural and socio­economic backgrounds; Ability to work in a culturally diverse setting with clients who are disabled, physically and chronically ill and/or who may have behavioral health issues; Ability to apply de-escalation and informal counseling techniques when dealing with patients/clients; Ability to interview patients/clients to evaluate their capabilities, needs, interests; Ability to read, comprehend and interpret information to communicate in a clear and concise manner; Ability to provide culturally appropriate educational information about health programs, social services and other resources to patients/clients; Ability to plan, direct and coordinate educational, recreational, therapeutic and work projects of participants and schedule patient/client appointments; Knowledge of and ability to interpret and apply office policies, laws, regulations (e.g.HIPPA) and procedures in a medical setting; Ability to use office equipment and computer systems/software programs; Ability to supervise and oversee the work of staff, and specific health programs; Ability to provide constructive feedback on work performance; Ability to communicate, compose and organize written information; Knowledge of and ability to evaluate data and information to make effective decisions.

Candidates must achieve a passing score on the multiple choice exam in order to continue in the selection process and will be placed on the confidential eligible list in rank order according to their final score.

Language Proficiency Exam: For positions requiring bilingual fluency, candidates deemed qualified who meet the special language condition must pass a departmentally approved language proficiency exam prior to appointment. Only those eligible candidates who pass the bilingual proficiency test will be considered for the bilingual position. Applicants must indicate on the application form the language(s) in which they claim proficiency.

Certification: The certification rule for the eligible list resulting from this examination will be the Rule of Ten Scores.
