Appointment Type

Temporary Exempt (TEX) pending permanent civil service appointment, this position is excluded by the Charter from the competitive Civil Service examination process and shall serve at the discretion of the Appointing Officer. This position is limited to 1,040 hours per fiscal year.

Through the provision of coordinated, compassionate, and high‐quality services, the Department of Homelessness and Supportive Housing (HSH) strives to make homelessness in San Francisco rare, brief, and one time. The Department provides assistance and support to homeless and at‐risk youth, adults, and families to prevent imminent episodes of homelessness and end homelessness for people in San Francisco. Services including outreach, homelessness prevention, emergency shelter, drop‐in centers, transitional housing, supportive housing, short‐term rental subsidies, and support services to help people exit homelessness. What We Do

San Francisco is a pioneer in homeless services and a leader in providing supportive housing as a permanent exit from homelessness. The Department of Homelessness and Supportive Housing (HSH), founded in 2016, is a national leader in the movement to end homelessness by developing a coordinated, equity-driven, client-focused system of compassionate services while piloting innovative models, and implementing best practice solutions with measurable results. HSH’s Homelessness Response System (HRS) oversees and implements a system of care that shelters, houses, and serves approximately 14,000 people daily. Major programs include: street outreach and service connection through the Homeless Outreach Team (SFHOT); 3,000-bed shelter system for adults and families including shelters for members of the LGBTQ community; rapid rehousing rental subsidies for families, adults, older adults, and transitional aged youth; the Problem Solving and flexible financial assistance programs; and robust supportive housing programs with nearly 10,000 units which provide permanent housing and services to formerly homeless individuals and families.

Role description

The 2587 Health Worker III with the Department of Homelessness and Supportive Housing (HSH) Housing Services Program provides case management services for the residents of the Star, Empress, Camelot, LeNain, Pacific Bay Inn, and Windsor Hotel. These hotels, located in the Tenderloin District, are permanent housing facilities that provide wrap-around services for formerly homeless individuals, many of whom have chronic medical and/or psychiatric illnesses. In addition to the incumbent, the staffing includes a program director, and one or two case managers; some locations have onsite registered nurse.

The position includes the following responsibilities:

  • Coordinates wrap-around services for the residents
  • Interviews potential new residents during the application process
  • Provides one-on-one counseling and education services
  • Demonstrates problem-solving skills
  • Provides community education and information about mental health and substance use issues and services
  • Provides client care in a manner that is both non-discriminatory and non-judgmental
  • Performs crisis intervention activities
  • Initiates and participates in group social and educational activities
  • Maintains clinical records and other necessary paperwork
  • Communicates effectively and collaborates with members of the inter-disciplinary team, primary care providers and members of the community
  • Assesses clients who may have substance abuse problems and mental health issues
  • Participates in weekly socialization activities with the residents which includes shopping for food at the Food Bank
  • Lifts boxes which may weigh up to 50 pounds
  • May include additional duties as assigned

How to qualify

Experience
Two (2) years of verifiable experience within the last five (5) years, working with a culturally diverse population performing a combination of at least two (2) of the following duties:

  • Serving as a liaison between targeted communities and healthcare agencies
  • Providing culturally appropriate health education/information and outreach to targeted populations
  • Providing referral and follow up services or otherwise coordinating care
  • Providing informal counseling, social support and advocacy to targeted populations
  • Escorting and transporting clients
  • Providing courier/dispatcher functions
  • Performing pre-clinical examinations of vital statistics, such as measuring a patient's weight, height, temperature and blood pressure

One year of full-time employment is equivalent to 2000 hours. (2000 hours of qualifying work experience is based on a 40-hour work week.) Any overtime hours that you work above forty (40) hours per week are not included in the calculation to determine full-time employment.

Desirable Qualifications

The stated desirable qualifications may be considered at the end of the selection process when candidates are referred for hiring

  • One (1) year experience working with people with severe mental illness in a residential setting or inpatient program
  • Comfort and familiarity working within a harm reduction model
  • Fluency in English and Spanish
  • Ability to work independently
  • Experience with facilitating group social and educational activities
  • Knowledge of primary care, psychiatric services and social services, and substance abuse treatment programs available to the urban pool in San Francisco
